If performance dropped and the employee did not feel they could perform at the higher grade as a manager, then a few months later discovered a medical condition that affected memory and performance, would that employee be able to go back and get pay retention due to the medical condition?

It is almost impossible to respond to a question of this nature without having access to the case file however, I will link you to the following CFR reference which defines exclusions for pay retention:
https://www.gpo.gov/fdsys/pkg/CFR-2011-title5-vol1/pdf/CFR-2011-title5-vol1-sec536-102.pdf
